We need all the information provided by your optician in your personal prescription to enable us to make your glasses exactly right for you.

Your optician has a legal obligation to provide you with a legible copy of your prescription, but don't worry if yours is difficult to read, contact us.

Your prescription will look similar to the example below.

It's important that you give us all the information, numbers and letters your optician writes on your prescription form. Some boxes may be left blank on your prescription, that's fine; just give us the information exactly as shown on your prescription.

When you come to order your glasses, remember to include everything on your prescription. The illustration above is just an example of what your prescription could look like; if yours has extra information not shown in the example above, you must enter it in the comments box after the prescription boxes.
